At Dixon Rehabilitation and Health Care Center, we want to celebrate your choices and your achievements. We create a plan to support your healing journey with your goals at the center. We collaborate with you, your physician and your family to design a plan to help you achieve the outcomes you want and reach the goals you set. Each recovery and rehabilitation plan at Dixon is customized to a specific individual and is carried out by our trained health care professionals and our Medical Director.

We provide a complete approach to recreation activities and encourage the development of a healthy lifestyle. Through our holistic, therapeutic wellness services, residents are able to understand their mental, emotional and physical health needs, while finding ways to gain strength and independence. In addition to physical therapy, our residents have access to services including exercise groups, pain management, diabetes management, skilled nursing care and memory care. For patients with heart conditions, our on-site cardiology consultants are an excellent resource. They provide education and are able to evaluate underlying causes of heart conditions and make suggestions for necessary lifestyle changes.

Our memory care team at Dixon Rehabilitation is warm and welcoming. We understand that our residents need their independence as much as they need our support. We encourage positive social interactions in our lovely indoor and outdoor meeting spaces and provide outlets for their creative energy through music, art and movement activities. Friends and family are always welcome to come for a visit or to share a meal!
